What changes at certification: our name appears on the Departments' certified-entity list and may be entered in the Notice of IDR Initiation. To be notified on that date, email disputes@trilatic.com with subject "notify" -- this is a mailbox, not a form; nothing is stored on our side beyond the message itself.
The initiating party names an entity in the Notice of IDR Initiation. The non-initiating party then has 3 business days to object and propose another entity; absent agreement, the Departments make the assignment (45 CFR 149.510(c)(1)).
Before any dollar amount, every dispute runs through nine eligibility rules: NSA applicability, state law, open negotiation, initiation window, batching, cooling-off, duplicate, wrong party, and missing information -- decided within 5 business days of final selection, with a 5-business-day cure period. For a fully-insured plan in a state with a specified state law (for example Texas), the Federal process does not apply and we say so at eligibility; self-funded ERISA plans that have not opted in remain federal. A batch may contain up to 50 qualified IDR items or services (45 CFR 149.510(c)(4)(i), June 2026 IDR Operations final rule, 91 FR 33949). Every surviving item's date of service must fall within a 30-business-day span of the earliest item. Each item must share one of three bases with another surviving item: the same patient encounter on consecutive dates (as the initiator asserts), the same or a comparable billing code, or -- for anesthesiology, radiology, pathology, and laboratory items -- the same Category I CPT code section. An item that fails all three is excluded; the rest of the batch proceeds.
Eligibility decided within 5 business days of final selection; median time to determination target of 20 business days; at least 95% of determinations issued within 30 business days. See Service commitments for the live figures against each.
Everything about a pending dispute happens on the portal record; we do not communicate with either party outside it. For a mis-keyed selection, email disputes@trilatic.com -- acknowledged within 1 business day.
Single dispute: $435 (within the 2026 range $200-$840). Batched dispute (1-25 line items): $545 (within $268-$1173). Each additional 25-line increment: +$150 (within $75-$250).
Worked example: a 1-line dispute is $435; a 25-line batch is $545; a 50-line batch is $695.
Each fee is within the range the Departments permit for 2026 under 45 CFR 149.510(e)(2)(vii) and is the same for both parties. No party is offered any discount, rebate, or volume arrangement, and the schedule is fixed for the calendar year.
