Eligibility is decided within 5 business days of final selection. Improper batches and state-law disputes are excluded, not waved through. Every written determination lists the factors weighed and the factors raised but excluded, with reasons. The fee is within the Departments' range and identical for both parties -- see below.
We file a conflict attestation -- clean, or a decline if one exists -- within 3 business days of selection, covering the preceding 12 months for every case-team member. A decline sends the dispute back for reselection. Either party may object to a named case-team member, with reasons, at any point before a determination issues; a late-discovered conflict is handled the same way, never issued around. Our quarterly resolver record breaks out every metric by how we came to be selected (initiator-named, payer-named after an objection, joint agreement, or random assignment), so a payer-named determination's outcome mix can be read separately from an initiator-named one.
Our case-management software drafts and screens for prohibited factors; a credentialed human independently reviews and signs every determination -- a model never issues one. The entity answers any inquiry from the Departments about this software's role in full.
Single dispute: $435 (within the 2026 range $200-$840). Batched dispute (1-25 line items): $545 (within $268-$1173). Each additional 25-line increment: +$150 (within $75-$250).
Worked example: a 1-line dispute is $435; a 25-line batch is $545; a 50-line batch is $695.
Each fee is within the range the Departments permit for 2026 under 45 CFR 149.510(e)(2)(vii) and is the same for both parties. No party is offered any discount, rebate, or volume arrangement, and the schedule is fixed for the calendar year.
