The CMS Federal IDR Public Use Files for 2025 Q3 and Q4 are the first to name the certified IDR entity on every determination. This report turns those two files, the CMS supplemental tables, the certified-entity list and the June 2026 Operations final rule into one sourced reference on the active entities: how an entity is named under the new rule (objection window, random selection, the five-business-day eligibility clock), what changes on 1 November 2026 for batching, the 2025 versus 2026 fee schedule, program- and state-level quarterly volumes on three published denominators, timeliness distributions, default-decision rates, entrants and exits since 2022, the recertification mechanics that first-cohort entities face in early 2027, and the $1.263 billion CMS reports paying certified entities in 2025 (CMS, Federal IDR public use file, 2025 Q3–Q4, Table 6).
Every entity-level figure carries its citation on the same line; where two computations disagree, both are shown. No rankings, no characterisations, no selection advice — facts a diligence team or counsel can cite as they stand.
Sold to healthcare law firms, diligence teams, consultants and certified IDR entities. Not sold to organisations that appear as a party or administrator in federal IDR disputes. Every purchaser is recorded on our counterparty ledger; if we are later certified, we recuse from any dispute involving a purchaser.
Hourly consulting and expert engagements on federal IDR process mechanics, the June 2026 rule, and the public-use-file data are available on the same terms: written opinions on the record, no contingent fees, every engagement logged on our counterparty ledger. Enquiries to research@trilatic.com.
